I understand and agree to the following by voluntarily participating in Blessings in a Backpack events:

1. I am physically able to take part in Blessings in a Backpack activities.
2. Risk of Injury.
   I know there is a risk of injury by voluntarily agreeing to participate in this event that involves physical activity and/or physical contact. I understand and accept that there are certain risks and hazards involved in participating in physical activities of lifting, bending and twisting that may result in injury or death to me or other participants.
3. Further, I, the undersigned participant, agree that in consideration for the right to participate in Blessings in a Backpack events and in consideration for permission to participate in the event arranged for by Blessings in a Backpack:
   • I voluntarily elect to accept and assume all risks of injury incurred or suffered by me (i) while practicing or playing as a member of the team so designated, (ii) while serving in a non-playing capacity as a team member during participation of the event or by other teams or by other players on my team, and (iii) while on or upon the premises arranged for by Blessings for practice or play.
   • I willingly agree to comply with the stated and customary terms and conditions for participation regarding these activities. If, however, I observe any unusual significant hazards during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
   • c. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless

Blessings in a Backpack and its affiliates, their officers, agents, servants, associations, employees, volunteers or any person or entity connected with the organization, or location, from any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by me.

Image and Likeness Release. I give permission to Blessings in a Backpack and its affiliates to use my likeness, photo, video, name, voice, words, and biographical information (“my likeness”) to acknowledge the sponsors’ and partners’ support for Blessings in a Backpack for which I understand I will not be compensated. Blessings in a Backpack and its sponsors and partners will not use my Likeness to endorse commercial products or services.

I have read, understand and voluntarily agree that this release is meant to bind me to the terms and conditions of this release which holds harmless Blessings in a Backpack and its affiliates from any and all responsibility and liability by my voluntary participation in this event.
